Diabetes Education
Diabetes affects more than just your health. It can change your diet, lifestyle and even your relationships. We created the Emanate Health diabetes education program to give people who are living with diabetes a safe, supportive place to tackle both the physical and mental challenges that come with the condition.
Our certified diabetes educators, registered dietitians and counselors are dedicated to giving our patients renewed hope and expert diabetes care. Here's a look at the diabetes education services we offer.
Diabetes education program
Our program is recognized by the American Diabetes Association (ADA) as meeting national standards for diabetes education and support. The program is offered at the Emanate Health Diabetes & Specialty Care Centers in both English and Spanish. A physician's order is required for enrollment.
When you begin the program, you'll attend three two-hour group classes specifically geared toward your level of diabetes. Over the course of the program you will:
- Get a personalized nutrition and fitness plan
- Receive a revised medication plan
- Learn behavioral modifications for long-term success
After the program ends, you'll continue with one-on-one follow-up appointments with a registered nurse, a dietitian and a counselor. Our staff is dedicated to helping patients meet their goals and has seen major success over the years. Patients who have gone through the diabetes education program have:
- Significantly reduced their A1C levels
- Learned how to better control their blood sugar levels
- Learned how to cope with stress and lifestyle changes
- Reduced their overall complications associated with diabetes
Diabetes education for pregnant women
We offer a specialized program for expectant mothers that's affiliated with the California Diabetes and Pregnancy Program "Sweet Success."
Sweet Success provides education to women dealing with either pre-existing diabetes or gestational diabetes. Education is available prior to conception and throughout pregnancy. All participants attend a four-hour group class and continue with one-on-one follow-ups throughout their pregnancy and postpartum.
Diabetes support groups
We welcome our patients and caregivers to attend our diabetes support groups. Our group meetings are held in both English and Spanish for both adults and adolescents.
Interfaith diabetes outreach
Have our team come to your place of worship and provide two hours of education.
