- If you are a patient requesting a copy of your own medical records, a legal guardian or a patient representative with a power of attorney, please visit the Patient Medical Records Request page.
- You are also able to obtain a copy of radiology/imaging CD and report done at Emanate Health Imaging at any of our hospitals’ Radiology/Imaging department nearest you.

All Copy Services must serve subpoenas for radiology CD and reports to the address below. We cannot accept a subpoena by fax. - SHARECARE

*Please be sure to complete and sign the form before sending.
Only the patient, parent/legal guardian, or the patient's legal health care representative can sign the form to release medical records. If you are requesting records on behalf of the patient or as the patient’s representative, please provide a copy of an Advance Directive/Durable Power of Attorney for Healthcare/Conservatorship.
Unsigned requests cannot be processed and failure to provide all necessary information may invalidate the request.
