We never really know what life may throw at us. For this reason, medical, dental and vision coverage may be extremely important to you and your family. Employees eligible to receive these benefits are those who are classified as working 48 hours or more per pay period. The following is a description of some of the health care benefits we offer employees:
EPO:
- Extremely competitive employee contribution rates
- Choice of tier 1 or tier 2 physicians
- Utilizes the Anthem Blue Cross PPO network
- Low co-payments
- Approved services at Emanate Health facilities are 100% covered
PPO:
- Extremely competitive employee contribution rates
- Choice of tier 1 (Emanate Health Medical Staff), tier 2 (Anthem Blue Cross Provider network physicians) or tier 3 (outside of Anthem Blue Cross network)
- Low co-payments
- Approved services at Emanate Health facilities are 100% covered
Delta Dental (DPO):
- Choice of dentists, in and out of network
- Low deductibles
- Preventive care covered at 100%
- Annual plan payment maximum
Dental (HMO):
- Provides care at little or no cost
- No deductible
- Preventive care covered at 100%
- Orthodontic benefit available
- No annual plan maximum
Vision:
- Choice of providers, in and out of network
- $10 co-pay
- Eye exams covered every 12 months
- Standard frames and lenses covered every 24 months
Flexible spending accounts
A flexible spending account (FSA) offers an employee the valuable opportunity to set aside money for certain qualified expenses, such as health care or dependent care. The money deducted from an employee's pay that goes into an FSA is not subject to payroll taxes, which may result in substantial payroll tax savings.

If you have loved ones who depend on your income for support, having disability benefits and life insurance can help protect your family's financial security.
Short-term disability
An injury or illness may leave us unable to work for a period of time. If this is a concern of yours and you would like to ensure you have the disability benefits you need in this situation, we offer a short-term disability plan.
Short Term Disability is 67% combined with State Disability Insurance.
Life insurance
Emanate Health offers several different options for life insurance coverage for our employees:
- Base coverage. At no cost to the employee, base coverage in the amount of one's base annual salary is available.
- Additional coverage. Up to three times one's base annual salary.
- Spouse and dependent coverage is available as well.
Accidental death and dismemberment
At Emanate Health, our employees and their families are important to us. We offer accidental death and dismemberment benefits in the following categories:
- Basic coverage. At no cost to the employee, up to $3,000 in coverage.
- Additional coverage. Up to 10 times an employee's annual base salary (up to $500,000).
- Spouse and dependent coverage is available as well.

Whether you wish to take a much-needed vacation or life has thrown you an unexpected curve ball, you may need time away from work.
The following are the benefits Emanate Health has to offer our employees if time away from work is needed:
- Paid time off (PTO) benefits to be used for vacations, holidays and sick time. The accrual of hours is based on length of employment and the hours worked in each pay period.
- Long-term sick (LTS) leave can be used starting on the eighth consecutive day of illness/accident or on the fourth day of workers compensation. LTS pay is integrated with California State Disability or workers compensation. The accrual is based on hours worked in each pay period.
- Bereavement pay can be used in the event of a death of a designated family member. You will receive pay for bereavement time if you are previously scheduled to work. A full-time, 8-hour employee may receive up to 32 hours of pay; a full-time, 12-hour employee may receive up to 36 hours of pay. Pay is prorated for part-time employees.
Employee assistance program
The employee assistance program (EAP) is a free, professional and confidential service provided for employees and family members for assistance in resolving personal problems. Some of the areas the EAP can help with are:

Planning for retirement is an important issue that everyone, no matter their age, may wish to consider. Emanate Health offers employees the following retirement benefits:
